Women’s Hormone Health Guidelines

Dr. Joseph J Collins RN, ND and Ann Collins are founders of Your Hormones, Inc. Dr. Collins has been directly involved in advancing the practice of natural hormone health since 1993. His functional endocrinology clinical practice focuses on adrenal fatigue, hypothyroidism, menopause, andropause, PMS/PMDD, PCOS and blood sugar disorders. Dr. Collins was one of the first doctors to measure testosterone levels in all his female patients and develop the 12 different Menopausal Types culminating in his book Discover Your Menopause Type.

Dr. Collins has extensive experience in the advanced use of adaptogenic herbs to support and improve hormone function through the modulation of homeostasis regulatory systems that control hormone production and hormone function. Dr Collins is a medical educator, author, and speaker who has been teaching at advanced medical seminars and conferences for many years on women’s hormonal health and herbal medicine. He has worked with or advised many functional diagnostic medical laboratories, and has contributed to the advances made in clinical applications of diagnostic testing. At a national and international medical educator, Dr Collins continues to educate and advise other integrative practitioners in the assessment and management of menopause, PMS/PMDD, thyroid and adrenal disorders, as well as inflammatory diseases, celiac disease, and other chronic conditions.

Phytocrinology

As a naturopathic physician, I’m pleased to announce that my clinic now has available the Dr. Collins’ Hormone Specific Formulation range of encapsulated herbal formulas highly targeted towards specific women’s hormonal imbalances. The Dr. Collins encapsulated range of 7 different (100 percent vegetarian) herbal products has been formulated exclusively by Douglas Laboratories to the highest possible standards. The phytocrine™ model of endocrine therapy was created by Dr. Joseph J Collins, based on many years of clinical herbal medicine/hormonal imbalance experience, and validated by extensive review of published international scientific/medical research. The word phytocrine™ was created by Dr. Collins in 2005 from phyto (plant) and crine (to secrete). “Phytocrinology is the study of hormone-like messages which originate from plant sources.” (Dr. Joseph J. Collins)

Protocol 242

Protocol 242 is a unique dosage guideline developed by Dr. Collins. It is based on traditional and current clinical observations concerning response to herbal therapy. Simply put; when using a potent herbal formulation it is best to start at a lower dosage (the initiation phase) before increasing to the full therapeutic dosage (the restoration phase), and then lowering the dosage to a maintenance dosage (the maintenance phase) once the goals have been accomplished.  To summarise Protocol 242: adults take 2 capsules each day with food for 1 to 2 weeksor as directed. The dose may then be increased to 4 capsules each day with food for 2 to 4 months or as directed. After 2 to 4 months dosage may be lowered back down to 2 capsules each day with food and may continue on that dosage as needed or as directed.

PMS and PMDD

I have been treating patients with PMS or PMDD for over 25 years.  The symptoms associated with premenstrual syndrome (PMS) and premenstrual dysphoric disorder (PMDD) may be caused by one or more different hormonal imbalances. One imbalance that now appears to be common to all cases of PMS & PMDD is the inadequate production of progesterone and allopregnenolone by the adrenal glands. Other causes may include excessive production of androgens such as testosterone and DHEA, poor thyroid function and problems with insulin & blood sugar function. The hormone health formulation designed by Dr. Collins that is first and foremost indicated for premenstrual syndrome (PMS) and premenstrual dysphoric disorder (PMDD) is ProgestoMend™, a unique blend of phytoprogesterones, adaptogens and other herbs to naturally support hormonal balance by primarily supporting the body’s natural ability to make progesterone and pregnenolone while also helping control androgen excess, helping support thyroid function, and supporting proper blood sugar function.

ProgestoMend™

jt_progestomendThe primary function of ProgestoMend™ is to support the natural production of progesterone in addition to supporting how tissues throughout the body respond to progesterone. This is accomplished by supporting the function of progesterone producing glands, and by supporting the function of progesterone responsive tissues through the use of herbs that have phytoprogesterone properties and herbs that mimic the actions of progesterone.Progesterone is produced in the adrenal glands of both genders, as well as by the ovaries and corpus luteum in females. While progesterone is also produced in the Leydig cells of the testes, it is typically converted to testosterone. The synergistic combination of specific herbs in ProgestoMend™ support important functions associated with optimal progesterone health through progesterone specific actions of these herbs which:

  • Supports the natural production of progesterone.
  • Support healthy adrenal function, a major site of progesterone production.
  • Promote progesterone response in target tissue through the use of herbs with progesterone receptor agonist activity.
  • Promotes healthy function of bone, heart, brain, breasts, uterus & other progesterone sensitive tissues with progesterone mimetic herbs.
  • Support the anti-inflammatory, antispasmodic and anxiolytic properties of progesterone with progesterone mimetic herbs.
  • Supports healthy brain function, memory, and cognition, enhances mood while promoting tranquillity.
  • Dosage – Dr. Collin’s Protocol 242
  • 100% Vegetarian – Made with animal-free with Vegetarian Capsules.
  • NO yeast, wheat, gluten, soy protein, milk/dairy, corn, sodium, sugar, starch, artificial coloring, preservatives or flavoring.

Menopause

A common mistake Dr Collins has noticed with many women who are trying to improve the function of their hormones, whether it is thyroid hormones, sex hormones, or adrenal hormones, is to go directly to bio-identical hormone replacement therapy (BHRT) before they give the body a chance to restore optimal hormone health on its own. BHRT is still “replacement” therapy – it assumes the body is not capable of healing itself, and making its own hormones again. Patients are generally advised that they will need replacement hormones “for the rest of their life”. Dr. Collins noticed that many female patients in particular inadvertently give up on allowing the body to heal itself, and settle for “replacement”. Some of the most compassionate and well educated physicians enable these patients in taking the easy road of “giving up”, and missing the opportunity of allowing their patients’ body to heal itself.

If the body needs more balanced nutrition, better essential fatty acids, and improvement of the digestion system, detoxification systems and the immune system, then skipping any of those steps and jumping directly into hormone replacement therapy, sets the body up for more problems, and the underlying problems with woman’s health persist. In many cases, those patients who went directly to bio-identical hormone replacement, feel good for a while, then start to have the underlying problems manifest as other symptoms that are difficult to alleviate because some of their symptoms are obscured by the hormones they are actually taking.

Many of these unfortunate patients “doctor hop” until they find a practitioner who starts to focus on the foundations of their health. Once the practitioner has addressed balanced nutrition, detoxification if required, and gives the patients herbs to help the hormones work better, the BHRT is able to keep them healthy. In some instances, patients really do need BHRT as soon as possible. Their body cannot recover the ability to make enough hormones. But these patients always do much better if they also address balanced nutrition, essential fatty acids, and use herbs to help the hormones work better. Dr. Collins has worked with many women over the years whom have gone directly to BHRT and still felt miserable, until he addressed their underlying foundations of health and in addition offered targeted herbal support.

In menopause we now recognize that some women may have low estradiol, while others continue to always have adequate amounts of estradiol. We also recognize that while some women truly are deficient in progesterone, other women continue to always have adequate amounts of progesterone. We also realize that while some women have testosterone deficiency, and others have adequate amounts of testosterone, there are some women who have excessive testosterone levels. These various patterns are discussed in Dr. Collins’ book Discover Your Menopause Type. These patterns can persist into the seventh decade of life and even beyond.

In menopause there are a number of possibilities:

  • Estradiol & progesterone are both adequate.
  • Estradiol is deficient.
  • Progesterone is deficient.
  • Estradiol & progesterone are both deficient.

Add to those four possibilities the fact that:

  • Testosterone may be normal.
  • Testosterone may be low.
  • Testosterone may be high.

Adrenal Fatigue, PMS and Menopause

imagejjjnsAdrenal hormones are important to healthy menstruation, and stress is a very well known factor when it a cause menstrual cycle dysfunction. The adrenal glands are the only source of testosterone in women, and after menopause are the only source of the female hormones, estrogen and progesterone. These and other adrenal hormones play a major role in menstruation, sexual function, physical and psychological wellbeing, and the aging process, among other things. When these hormones are low, a number of unpleasant physical and psychological symptoms can occur. It has been observed clinically that many women experiencing adrenal fatigue also experience PMS and tend to have difficulty during peri-menopause and menopause.

When a younger woman is under considerable stress, trying to raise her children and live a healthy and well-balanced lifestyle, she may experience PMS. In addition to making the appropriate diet and lifestyle changes, her adrenal glands are best supported during these stressful years, as balanced adrenals can significantly improve her ability to cope more with the stress of living in this busy time of her life. Strong adrenal function will ensure good levels of energy and a great sleep function, balanced moods and good levels of cognitive function.

When a middle aged women enter peri-menopause and menopause, her ovaries slow their production of estrogen and progesterone, and the adrenal glands have to pick up the slack for menopause to proceed smoothly. If the adrenals are already fatigued, it may be more difficult to meet this extra demand for hormone production. In fact, the adrenal glands may become even more depleted as a result, creating a vicious cycle.

Disturbed sleep, insomnia and/or pronounced morning fatigue may be a tip-off that low adrenal function is a factor in PMS or menopausal discomfort. A saliva cortisol/DHEAS test that measures adrenal hormones at four different times in the day can help determine whether adrenal function is a contributing factor. Support for healthy adrenal function can often provide help in this situation.
